Enrollment for candidates qualified in the admission procedure will be held from 28\/07\/2021 to 30\/07\/2021 between 8:30 and 12:30 and 13:30 – 15:00 (on Friday, 30\/07\/2021 only until 12:00).<\/p>\n

The enrollment should be made in person or by a person authorized in writing (the authorization remains in the documentation) at the seat of the Doctoral School, Katowice, ul. Bankowa 14, room 406 (foreigners, room 406<\/span><\/strong>).<\/p>\n

Candidates from abroad, who cannot come to Poland shortly after admission results (for visa or epidemic reasons), should contact Ms. Diana Pasek in the Doctoral School’s office (diana.pasek@us.edu.pl<\/a>).<\/p>\n

Candidates who have not yet uploaded a photo to the IRK system and have not filled in the ORCID number are requested to fill in immediately – without this enrollment will not be possible.<\/p>\n

Please cover your mouth and nose while on school grounds and use your own pen.<\/p>\n<\/div>\r\n <\/div>\r\n <\/div>\r\n

Required documents<\/h3>\r\n

The candidate brings the documents in a white, tied A4 folder. The folder should be described in accordance with the instructions (za\u0142 1 opis teczki<\/a>). The folder must contain:<\/p>\n

    \n
  1. Application \u2013 personal survey \u2013 for admission to the Doctoral School, printed from the personal candidate’s account (IRK)<\/li>\n
  2. A copy of the diploma confirming obtaining the second-degree qualifications (the original is available for control), or in the absence of such a document, a certificate from the dean’s office confirming the award of the title. A copy of the diploma should be delivered immediately after receiving it.<\/li>\n
  3. Completed in accordance with the instructions, printed in two copies ZUS ZUA file (za\u0142 2 druk ZUS ZUA<\/a>; za\u0142 nr 3 ZUS ZUA obja\u015bnienia<\/a> – for candidates who don’t speak Polish – please bring an empty document, we will help you to fill it).<\/li>\n
  4. A completed scholarship application (za\u0142 4 application for a doctoral scholarship 2021<\/a>) printed in two copies<\/strong>.<\/li>\n
  5. A signed declaration of knowledge of the principles resulting from the Regulations for the protection, use and management of the results of intellectual work at the University of Silesia in Katowice and the principles of their commercialization (5 – o\u015bwiadczenie o znajomo\u015bci zasad<\/a> – only in Polish for now).<\/li>\n
  6. Signed oath (za\u0142 6 \u015blubowanie – oath<\/a>).<\/li>\n
  7. A statement authorizing the University of Silesia to demonstrate the achievements of the doctoral student in the evaluation of the discipline (za\u0142 7 – declaration for a discipline<\/a>).<\/li>\n
  8. Information clause (za\u0142 8 – informational clause GDPR<\/a>).<\/li>\n<\/ol>\n
