Announcement on vaccinations in the workplace (University)

REGISTRATION FOR COVID-19 VACCINATION THROUGH THE UNIVERSITY OF SILESIA FOR EMPLOYEES, STUDENTS AND DOCTORAL STUDENTS

Starting today, it is possible to register for vaccination organised by the employer in line with the announcement of the Ministry of Health on the organisation and implementation of vaccination in workplaces.

We have no information on the type of vaccine or dates when the vaccinations organised under the programme for employers will take place. The date when vaccination in workplaces begins depends on the availability of vaccines and is expected at the turn of May and June.

The individuals who already have the vaccination date set in accordance with the general registration system through National Health Fund (NFZ) may also report for vaccination through the University of Silesia, and ultimately choose the more convenient date.

 

HOW TO REGISTER A MEMBER OF THE ACADEMIC COMMUNITY FOR VACCINATION THROUGH THE UNIVERSITY OF SILESIA:

 

  • The registration form to register for vaccination through the University of Silesia will be active until Monday, 10 May 2021, 11:59 p.m. It will not be possible to register after this date.
  • Members of the academic community – employees, students, doctoral students and cooperating individuals can register at: https://formularze.us.edu.pl/szczepienia_covid, choosing the option Registration for COVID-19 vaccination.
  • Log in (in the same way as to USOSWeb system).
  • For the purpose of registration, provide the following data: e-mail address, first name, surname, PESEL number (or passport number), and telephone number (based on the GDPR clause included in the registration form).
  • Specify the preferred vaccination location in the registration form. There are 4 vaccination points in Katowice, 1 vaccination point in Chorzów, 2 vaccination points in Sosnowiec and 2 vaccination points in Cieszyn available. The ultimate vaccination point depends on the number of applicants in a particular vaccination point, dates and effectiveness of vaccination points.
  • If you have any technical problems or need support, write to: szczepienie_covid@us.edu.pl. The requests are processed on a first come, first served basis.
  • Further information will be provided on an ongoing basis by e-mail or directly through the vaccination point upon launching the vaccination programme by the University of Silesia, which depends on the availability of vaccines.

REGISTRATION FOR COVID-19 VACCINATION THROUGH THE UNIVERSITY OF SILESIA FOR FAMILY MEMBERS OF EMPLOYEES, STUDENTS AND DOCTORAL STUDENTS

According to the announcement of the Ministry of Health on the organisation and implementation of vaccination in workplaces by the employer (University of Silesia), it is also possible to register for vaccination family members of employees, students, doctoral students and other individuals cooperating with the University of Silesia. The vaccination procedure and rules are the same as for the members of the academic community, however, family members are registered through the employee, student or doctoral student.

HOW TO REGISTER A FAMILY MEMBER FOR VACCINATION THROUGH THE UNIVERSITY OF SILESIA:

 

  • The registration form to register for vaccination through the University of Silesia will be active until Monday, 10 May 2021, 11:59 p.m. It will not be possible to register after this date.
  • Members of the academic community – employees, students, doctoral students and cooperating individuals can register their family members at: https://formularze.us.edu.pl/szczepienia_covid, choosing the option Registration for COVID-19 vaccination for family members – STAGE 1.
  • Log in (in the same way as to USOSWeb system) .
  • In the registration form you have to provide the e-mail address of the family member who wants to register for vaccination with the University of Silesia in Katowice.
  • After clicking Send application, access code will be sent to the provided e-mail address to enable registration of your family member.
  • The family member has to click the link received in the e-mail or at: https://formularze.us.edu.pl/szczepienia_covidand choose the option Registration for COVID-19 vaccination for family members – STEP 2.
  • It will be possible to register the family member after entering the access code received by e-mail. The following data will have to be provided: e-mail address (the same as the one the access code was sent to), first name, surname, PESEL number (or passport number) and telephone number.
  • Specify the preferred vaccination location in the registration form. There are 4 vaccination points in Katowice (one of them at the Katowice Campus), 1 vaccination point in Chorzów, 2 vaccination points in Sosnowiec and 2 vaccination points in Cieszyn available. The ultimate vaccination point depends on the dates and effectiveness of vaccination points.
  • If you have any technical problems or need support, write to: szczepienie_covid@us.edu.pl. The requests are processed on a first come, first served basis.

VACCINATION WITH THE SECOND DOSE OF ASTRAZENECA VACCINE FOR ACADEMICS AND OTHER INDIVIDUALS TEACHING CLASSES AT THE UNIVERSITY OF SILESIA

In the last few days we were informed that the second vaccination dose was not carried out as initially planned in some vaccination points. Unfortunately, the University of Silesia has no influence over implementation of vaccination on particular dates, and some vaccination points regard the date specified during the first dose as indicative only, to be confirmed later. Depending on the specific vaccination point, the information on date confirmation is provided by SMS, MMS or phone, in the same way as before the first dose. However, we know that some vaccination points carry out vaccination at the initially specified date without confirming it (e.g. the vaccination point in Ochojec). Although it may be misleading, we have no influence over it. We have asked all vaccination points either to confirm the date, or to postpone it in advance, if necessary. Communication regarding the date and other aspects of visit in a vaccination point takes place directly between the patient and the specific vaccination point. The University does not facilitate such arrangements. According to the latest recommendations of the Ministry of Health, in order to be most effective, AstraZeneca vector-based vaccine should be administered at intervals of 12 weeks between two doses.
