Wymagane dokumenty

Pod rygorem utraty miejsca na liście kwalifikacyjnej, kandydat, który został zakwalifikowany do przyjęcia na studia, jest zobowiązany do dokonania wpisu na studia oraz złożenia wymaganych dokumentów, w opisanej teczce (wzór opisu teczki), w dziekanacie odpowiedniego wydziału, w miejscu i terminie wskazanym przez Wydziałową Komisję Rekrutacyjną w systemie IRK przy kierunku.

Przy składaniu dokumentów wymagane jest okazanie dowodu tożsamości lub paszportu.

Jeżeli po otrzymaniu informacji o zakwalifikowaniu na I rok studiów kandydat nie będzie mógł osobiście dokonać wpisu i złożyć dokumentów, może zrobić to w jego imieniu druga osoba, upoważniona przez kandydata. Osoba ta musi posiadać pisemne upoważnienie, podpisane przez kandydata (wzór upoważnienia).

Uwaga! Kandydat, który nie dopełni ww. formalności w wyznaczonym terminie, nie zostanie przyjęty na studia.

Undergraduate programmes and long-cycle MA/MSc programmes Undergraduate programmes and long-cycle MA/MSc programmes
    • a personal data form including your photo: printed from the IRK system;
    • a xerox copy of your matura certificate, IB certificate, EB certificate or an equivalent certificate from abroad. Bring both the original certificate and the copy to the dean’s office, a university employee who accepts your documents will officially confirm that the copy you submit is identical with the original.
    • Candidates who possess a supplement to their matura certificate or an official document specifying the results for each subject will also have to submit it. Bring both the original certificate and the copy to the dean’s office, a university employee who accepts your documents will officially confirm that the copy you submit is identical with the original.
    • 1 photo in the electronic form. The candidate has to upload his/her photo to his/her candidate’s account in the IRK system after s/he is accepted for the programme of his/her choice and then print the personal data form including the photo;
    • doctor’s certificate: only for the programmes that require candidates to undergo medical exams;
    • authorisation to submit documents – if the candidate’s documents are submitted by a third party. If documents are submitted on behalf of the candidate by a third party, that party should provide the University employee with an personal authorisation issued and signed by the candidate for that party. ATTENTION! Documents printed from the personal registration account in the Online Application System must be signed by the candidate’s hand (the authorisation does not entitle to sign on behalf of the candidate); authorisation template;
    • consent to study of a minor candidate – a declaration of the statutory representative(s) signed separately by each statutory representative; consent template;
    • nuns and monks applying for theology submit the consent of their direct superior;
    • winners and finalists of scientific “Olympics” at the national level who are exempt from the normal qualification procedure submit a certificate confirming the position that they won;
    • winners of international and national contests who are exempt from the normal qualification procedure submit a certificate confirming the position that they won.
Graduate programmes Graduate programmes
    • a personal data form including your photo: printed from the IRK system;
    • a xerox copy of your BA, BSc, BEng, MA, MSc or MEng diploma. Bring both the original diploma and the copy to the dean’s office, a university employee who accepts your documents will officially confirm that the copy you submit is identical with the original.
    • a xerox copy of the supplement to your BA, BSc, BEng, MA, MSc or MEng diploma, if it is required for the qualification procedure but has not been submitted at earlier stages of the admission process. Bring both the original document and the copy to the dean’s office, a university employee who accepts your documents will officially confirm that the copy you submit is identical with the original.
    • a certificate confirming the average calculated from the grades you received issued by the authorities of the university/college you graduated from, if the average is one of the admission criteria (to be submitted if the average is not specified in the supplement to the diploma);
    • 1 photo in the electronic form. The candidate has to upload his/her photo to his/her candidate’s account in the IRK system after s/he is accepted the programme of his/her choice and then print the personal data form including the photo;

Candidates who possess a secondary school-leaving certificat(...) Candidates who possess a secondary school-leaving certificate or a university diploma from abroad

Candidates who possess a secondary school-leaving certificate or a university diploma from abroad submit the documents enumerated above plus the following:

    • a xerox copy of an administrative decision confirming that the document entitles the candidate to continue his/her education also at university level, if his/her right to study at university level is not stipulated under the relevant legal regulations or international agreements. Bring both the original document and the copy to the dean’s office, a university employee who accepts your documents will officially confirm that the copy you submit is identical with the original. You can also apply to the Rector of the University of Silesia to prolong the deadline of submitting this decision until the end of the first semester, or even a later date, if necessary;
    • certification in the form of a legalization or an apostille, if the document was issued by an institution within the education system of a country that is a party to the Hague Convention of 5 October 1961 abolishing the legalization requirement for foreign official documents;
    • a xerox copy of a document stating that your secondary school-leaving certificate or university diploma entitles you to start or continue studying at university level in the country where it was issued, issued or certified by the school or educational establishment that you attended, by the educational authorities of the country where it was issued, or by any other body having the authority to issue such documents, if your right to study at university level is not specified in the certificate/diploma and it is not stipulated under the relevant legal regulations or international agreements. Bring both the original document and the copy to the dean’s office, a university employee who accepts your documents will officially confirm that the copy you submit is identical with the original;
    • certified translation into Polish of the certificate or diploma and the above-required documents (if they were drawn up in languages other than Polish or English).
    • a document confirming the English language proficiency – in the case of studies undertaken in English. It is allowed to submit a photocopy of the document after it has been authenticated on site by an employee of the University on the basis of the original document presented by the candidate. Documents from the qualification procedure also confirm the English language proficiency.
    • a document confirming the Polish language proficiency – for studies conducted in Polish. The following documents are honoured:
      • a secondary school-leaving certificate issued in the Republic of Poland, which entitles you to apply for admission to studies,
      • diploma of higher education, conducted in Polish,
      • a diploma of higher education in Polish Studies, or Applied Linguistics in Polish language,
      • a certificate, diploma or other document confirming the completion of a secondary school outside Poland, where all classes were conducted in Polish; applies only to a document that entitles you to apply for admission to studies in the country of issue of this document,
      • a document confirming the completion of a one-year preparatory course for undertaking studies in Polish in authorised units (i.e.: selected by the Polish National Agency for Academic Exchange (NAWA) or previously designated by the minister responsible for higher education) at least at the B1 language proficiency level,
      • certificate of the Polish language proficiency confirming knowledge of the Polish language at least at the B1 language proficiency level, issued by the State Commission for the Certification of Proficiency in Polish as a Foreign Language,
      • a document confirming the Polish language proficiency at the level allowing for studies in Polish, issued by the School of Polish Language and Culture (SJiKP) of the University,
      • an IB or EB diploma with the result of the Polish language exam

Candidates who are not Polish citizens additionally have to submit:

    • a document confirming the basis on which you should be admitted and study at the University of Silesia (e.g. the decision by the head of NAWA, for scholarship holders);
    • the candidate’s statement that s/he possesses documents confirming the basis of being exempt from fees, specified in Article 79 Section 1 Points 3 and 5 of the Act on Higher Education and Science of 20 July 2018 (Journal of Laws 2018, position 1668). When submitting this statement, you need to show the original documents at the dean’s office;
    • the candidate’s statement that s/he possesses the Polish Card (Karta Polaka). When submitting this statement, you need to show the original document at the dean’s office.

If, for any reason, you decide not to study the programme you have applied for, please submit a written resignation statement (see specimen).
The statement should be submitted immediately after you receive the information that you have been accepted for the programme that you decide not to attend. Remember that there are still candidates on the waiting list who may want this place!
You can submit this statement in person at the relevant dean’s office, send it by regular mail to the faculty’s postal address or send a scan of it by e-mail to the Faculty Admission Commission (see ‘Kontakt do komisji’ on the list of accepted candidates).
