Insurance

The University of Silesia submits for health insurance all its Polish-origin students who completed the ag of 26 and who do not possess any other source of insurance coverage (i.e. employment contract, conduct of economic activity or who are not insured by the spouses).

If a student is not above the age of 26, he or she should  be covered by insurance as family member by one of his or her parents or grandparents or legal guardians or if they entered a marriage – by their spouse. If neither of these persons can insure them, then the university may do so post reviewing the specific case of student’s family situation.

The basis for submission for health insurance of a given student by the University of Silesia is the submission by him or her in the relevant dean’s office of a declaration on not being covered by any health insurance from another source and filling out and signing documents related to the submission to the Social Insurance Institution.

Students submitted to health insurance by the University of Silesia are obliged to submit for health insurance all family members who are not covered by health insurance from another source.

Students who are the citizens of a European Union member state or country from the European  Free Trade Association (EFTA) residing in the territory of Poland may use healthcare on the same principles as the insured Poles, provided that they hold a valid European Health Insurance Card issued in their country of origin.

Students who are not residents of member states of the European Union or members states of the  European  Free Trade Association (EFTA) and who cannot demonstrate a valid Pole’s Card or a document that certifies that they have been considered as persons with Polish origins in the meaning of the provisions on repatriation may conclude with the National Health Fund (NFZ) an agreement for voluntary healthcare insurance and pay monthly contributions independently.

Students who are not citizens of member states of the European Union or members states of the  European  Free Trade Association (EFTA) and who study in Poland and possess a valid Pole’s Card or have been considered as persons of Polish origin in the meaning of the provisions on repatriation and are not subject to the obligation of health insurance on another account may have their health insurance contributions paid for by the University of Silesia. For this purpose, they must conclude an agreement for voluntary health insurance with the National Health Fund (NFZ) and they must visit the dean’s office with such an agreement or document confirming its conclusion issued by NFZ in order to fulfil further insurance formalities.

Students submitted to health insurance by the University of Silesia are obliged to immediately inform the dean’s office about the appearance of another title to the health insurance (such as employment contract, contract of mandate) and regarding the change of data of themselves or of family members they submitted to their health insurance.

More information regarding health insurance and printouts of the forms may be found on the website of the University of Silesia us.edu.pl/student/wsparcie-i-fundusze/ubezpieczenie/ubezpieczenia-zdrowotne.
