Health insurance

I.Students who are Polish citizens (the below information also applies to the PhD students who began their PhD studies before the academic year 2019/2020)

  1. A student is subject to compulsory health insurance as a family member of an insured person:
  • below the age of 26 – parents, grandparents or legal custodians are obliged to report the student for health insurance;
  • if the student has a significant degree of disability or equivalent (regardless of age) – parents, grandparents or legal custodians are obliged to report the student for health insurance;
  • if the spouse is subject to compulsory health insurance – the spouse is obliged to report the student for health insurance.
  1. University of Silesia reportsfor health insurance and pays premiums for the student who:
  • is over 26 years old and is not subject to compulsory health insurance otherwise;
  • is under 26 years old and is not subject to compulsory health insurance otherwise, in particular when the student does not have parents, grandparents, legal custodians or spouse, or such persons are not subject to compulsory health insurance and the student does not receive a family pension.
  1. The students reported for health insurance by the University of Silesia are obliged to report for health insurance all family members who are not subject to compulsory health insurance otherwise.
  2. Submitting (in person or by post) a statement on not being subject to health insurance otherwise in the relevant dean’s office plus completing and signing the application documents to ZUS (Social Insurance Institution) – ZZA and optionally ZCNA forms – are the grounds for reporting a student for health insurance by the University of Silesia.   3. Student’s compulsory health insurance expires on the day of completing studies or being expelled from studies.
  4. Student’s right to health care for students expires after 4 months from the day of completing studies or being expelled from studies.
  5. The right to health care for the student’s family members ceases after 30 days from the expiry date of compulsory health insurance.
  6. The student reported for health insurance by the University of Silesia shall immediately inform the dean’s office on the arising of a different insurance coverage(e.g. employment contract, contract of mandate, business activity) or a change of their own personal data or the data of their family members reported for health insurance.
  7. The document that confirms reporting for health insurance is issued in the relevant dean’s office.

II. Foreign students who are EU citizens.

The students who are citizens of an EU or EFTA member state – a party to the European Economic Area Agreement, residing in the territory of this state, can use health care based on the same rules as the Polish insured students, provided that they have the valid European Health Insurance Card (EKUZ) issued in the state of origin.

III. Foreign students who are not EU citizens and do not have the Pole’s Card.

The students who are not citizens of an EU or EFTA member state – a party to the European Economic Area Agreement, and cannot demonstrate a valid Pole’s Card or other document confirming that they are recognised as individuals of Polish origin within the meaning of the provisions on repatriation, may enter into an agreement for voluntary health insurance with the National Health Fund (NFZ) and pay a monthly premium individually at the Social Insurance Institution (ZUS).
To arrange a health insurance, you need to go to the Silesian Branch of the National Health Fund (NFZ) in Katowice, ul. Kossutha 13 (8.00–15.00) in order to sign the agreement for voluntary health insurance. Information is available on the NFZ website. In order to complete any remaining formalities, you need to go to the Social Insurance Institution (ZUS).

Required documents:

  • student status certificate from the relevant dean’s office or secretary office of doctoral studies;
  • passport;
  • document confirming residence in the Silesian Voivodeship;
  • PESEL number.

IV. Foreign students who are not EU citizens and have the Pole’s Card.

The students who are not citizens of an EU or EFTA member state – a party to the European Economic Area Agreement, study in Poland and have a valid Pole’s Card or have been recognised as individuals of Polish origin within the meaning of the provisions on repatriation, and are not subject to compulsory health insurance otherwise, may be reported for health insurance by the university.

Submitting (in person or by post) a statement on not being subject to health insurance otherwise in the relevant dean’s office plus completing and signing the application documents to ZUS ZZA are the grounds for reporting a student for health insurance by the University of Silesia.

V. In order to use free-of-charge health care, you need to complete the relevant statement in any clinic which has an agreement signed with the National Health Fund (NFZ).

VI. Retroactive reporting for health insurance

If a student is not reported for insurance health, although they have the right to it, and thy have used health care benefits, they may be reported retroactively.

Dates.

You can be reported retroactively for health insurance:

  • within 30 days from the date of using health care benefits (the written confirmation of using health care benefits should be submitted at the relevant dean’s office),
  • within 30 days from the moment when the National Health Fund notifies the student that they began the procedure of claiming the costs of such health care benefits (the letter from the National Health Fund should be presented for inspection at the Dean’s Office).

If the student reports retroactively for insurance within this period, they will not incur the costs of treatment within the National Health Fund (NFZ).
