Health insurance

I. Students who are citizens of the Republic of Poland (the following information also applies to doctoral students who started their doctoral studies in the academic year 2019/2020)

  1. The student shall be covered by the health insurance as a family member of the insured person:
  • up to the age of 26 – the obligation to apply for health insurance shall fall to the parents or legal guardians;
  • if the student has a certificate of significant disability or other of equivalent status (regardless of age) – the obligation to apply for health insurance shall fall to the parents or legal guardians;
  • in a situation where the spouse is covered by the compulsory health insurance, the obligation to apply for insurance shall fall to the spouse.
  1. The University of Silesia applies for health insurance and pays the premium for a student who:
  • is 26 years old or older and is not covered by any other health insurance;
  • is under 26 years old and is not covered by any other health insurance, in particular if he or she has no parents, legal guardians or spouse, or these persons are not covered by any other health insurance obligation and when the student does not receive a survivor’s pension.
  1. Students registered for health insurance by the University of Silesia are obliged to apply for health insurance all family members who are not covered by other health insurance.
  2. The basis for applying for student health insurance by the University of Silesia is submitting to the relevant dean’s office a declaration of not being covered by other health insurance, as well as completing and signing the application documents to the Social Insurance Institution.
  3. The student’s health insurance obligation expires on the day of graduation or being expelled from the list of students.
  4. The right to the student health care expires after 4 months from graduation or being expelled from the list of students.
  5. The right to the health care for student’s family members expires after 30 days from the date of expiry of the health insurance obligation.
  6. A student registered for health insurance by the University of Silesia is obliged to immediately inform the dean’s office about the occurrence of any other health insurance obligation (e.g. employment contract, commission contract) and about any change of the personal data of the student or the student’s family members applied for health insurance.
  7. The document confirming the application for health insurance (data update form, famIly information form) is issued at the dean’s office, the document confirming the payment of premiums (monthly information for the insured person) is issued at the Student Social Affairs Office.

II. Foreign students who are EU citizens

Students who are citizens of a Member State of the European Union or a Member State of the European Free Trade Agreement (EFTA) – parties to the agreement on the European Economic Area who reside in the territory of this country can benefit from health care on the same terms as Polish insured persons, provided they have a current European Health Insurance Card (EHIC) issued in the country of origin.

III. Foreign students who are not EU citizens and do not have the Polish Card

Students who are not citizens of European Union Member States or a Member State of the European Free Trade Agreement (EFTA) – parties to the agreement on the European Economic Area and not having a valid Polish Card or a document certifying that they have been recognized as persons of Polish origin within the meaning of repatriation regulations, may conclude a voluntary health insurance contract with the Polish National Health Fund (NFZ) and pay the monthly premiums themselves.

To apply for the health insurance, go to the Silesian Provincial Branch of the Polish National Health Fund (NFZ) in Katowice, ul. Kossutha 13 (8.00 AM – 3.00 PM, phone number +48 32 735 1834) to sign a voluntary health insurance contract. Required documents:

  • certificate from the dean’s office or doctoral studies secretary on student status;
  • passport;
  • document confirming the registered residence in the Silesian Province;
  • PESEL number [Polish Resident Identification Number].

IV. Foreign students who are not EU citizens and have the Polish Card

Students who are not citizens of a Member State of the European Union or a Member State of the European Free Trade Agreement (EFTA) – parties to the agreement on the European Economic Area, who study in Poland and have a valid Polish Card or have been recognized as persons of Polish origin within the meaning of repatriation regulations and are not covered by any other compulsory health insurance can voluntarily insure themselves by concluding an appropriate contract with the Polish National Health Fund (NFZ). In such a situation, the health insurance premium is paid by the University of Silesia.

To apply for health insurance:

  1. Go to the Silesian Province Branch of the Polish National Health Fund (NFZ) in Katowice, ul. Kossutha 13 (8.00 AM – 3.00 PM, phone number +48 32 735 1834) to sign a voluntary health insurance contract.

Required documents:

  • certificate from the dean’s office on the student status;
  • the Polish Card or document confirming Polish origin within the meaning of repatriation regulations;
  • passport;
  • document confirming registered residence in the Silesian Province;
  • PESEL number [Polish Resident Identification Number].
  1. The student should report to the dean’s office in order to submit further formalities for health insurance application with the Polish Card or a document confirming Polish origin within the meaning of the provisions on repatriation and the contract with the NFZ or other document issued by the NFZ.
  2. If the application at the dean’s office was made on the basis of a document issued by the NFZ, a contract with the NFZ should be delivered immediately after receiving it.
  3. On the completion of the abovementioned formalities, the University of Silesia will pay monthly health insurance premiums.
  4. In order to benefit form the free healthcare, the student must complete the declaration at any clinic that has signed a contract with the NFZ.
