To submit an application to the Ethics Committee, please:

The application may not work properly in Safari. So far, there have been no problems with Chrome.

Please note!

• Fields marked with * are mandatory.
• Before filling in the form, we advise you to read the explanations provided next to them, as well as the guide.
• If multiple answers are possible, this is indicated in the given point.
• Individual sections of the application appear on the screen depending on the answer given (e.g., selecting NO to the question about the participation of vulnerable persons in the research means that the section devoted to this issue will not appear on the screen and does not need to be filled in).
• The application is saved automatically. To delete the data, press … in the upper right corner (next to the book-shaped icon “immersive reader”) and select “clear form.”

Please note!

• After submitting the application to the Committee, we suggest keeping a copy of it in PDF format. This may facilitate the work if the application needs to be corrected.
• A copy of the submitted application can be saved by clicking … in the upper right corner of the page that appears after submitting the application “Thank you for submitting your application to the Ethics Committee of the University of Silesia.”

 

If the application has received a conditional opinion or requires correction, please:

• fill out the application again
• in the final section, please respond to the EC’s comments and describe the changes made or attach a file containing such information
• send the application

After sending the application to the Committee, we suggest saving a copy in PDF format.
